Step 4: Implement

Prior to implementing your adapted innovation, acquire formal approval of your study through Institutional Review Boards, Research Review Committees, or other regulatory bodies, and seek additional approvals from community members, leaders, and implementers.

As the ADOPT-HF project was implemented, the study team interviewed health care providers and patients to learn about the barriers and facilitators to rural heart health care. The data from these interviews helped the study team and implementing partners at primary care clinics plan the implementation strategy for ADOPT-HF, and the rollout of the ADOPT-HF toolkit.

In fall 2025, the ADOPT-HF team launched a Project-ECHO series to on heart failure management in primary care, as part of the implementation strategy supporting the adaptation of the toolkit from hospital to primary care settings.
