Take a systematic approach to adapting an innovation.
How “transferable” is the innovation you would like to adapt? Culture, political systems, health care delivery models, geography, and many other factors affect the likelihood of success for transferring a health innovation from one setting to another. There are different frameworks and tools to help with this process. The field of implementation science provides insights into processes and methods for adapting an evidence-based practice in a new setting.

For the ADOPT-HF project, the study team used human-centered design tools, including empathy maps and journey maps, to help them understand the perspectives of patients living with heart failure and the health providers who provide care to them. The tools informed the design team as they adapted the ENGAGE-HF and OPT-HF toolkits for implementation at rural-serving US clinics. The toolkits developed in urban settings (ENGAGE-HF and DOT-HF) were provided to patients at hospital discharge. The ADOPT-HF toolkit, which includes many elements of ENGAGE-HF and OPT-HF, was adapted for primary care settings.
